HIPAA Notice of Privacy Practices (NPP)
This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. (Required by 45 C.F.R. §164.520; HHS also publishes model NPPs.) eCFR+1
Our Duties
We are required by law to: maintain the privacy of your protected health information (PHI); provide you this Notice; follow the Notice’s terms; and notify you following a breach of unsecured PHI. eCFR
How We May Use and Disclose PHI (examples)

Health Care Operations (e.g., quality review, training, audits).

As Required by Law (e.g., public health, abuse/neglect reporting, health oversight, law enforcement in limited cases).

Uses/Disclosures Requiring Authorization
Most uses of psychotherapy notes, marketing, or sale of PHI require your written authorization. You may revoke an authorization in writing. eCFR
